Role Overview:

The After hours Registered Nurse provides comprehensive hospice care during evenings and weekends, focusing on patient needs assessment, crisis response, and in-home visits.

• Triages patient needs via phone and responds to urgent calls
• Performs initial evaluations and determines hospice eligibility within 48 hours
• Conducts after hours visits including admissions, recertifications, death, and routine care
• Assesses and reassesses physical, psychosocial, and environmental needs at least every 15 days
• Documents care plans, interventions, and patient responses
• Provides care regardless of patient's background or home situation

• Unrestricted RN license in the practice state
• Valid driver’s license, insurance, and reliable transportation or public transit
• Preferred: CPR certification and 1+ years of clinical experience

Salary ranges from $67,104 to $100,656 annually, based on experience and market factors.
