Home Care Nurse LPN or RN

We’re seeking a caring, motivated LPN or RN who loves connecting with people, coaching caregivers, and helping seniors thrive safely in their homes. This is a hands-on, field-based role that blends nursing knowledge with relationship building and training.

What You’ll Do

• Conduct client assessments and create or update individualized care plans.

• Provide training, coaching, and delegation for caregivers in the field.

• Collaborate with families and the care team to ensure high-quality, compassionate service.

• Support medication oversight and related documentation as needed.

• Communicate effectively with caregivers, clients, and office staff to keep care seamless.

• Serve as a trusted nursing resource for caregivers and clients.

What We’re Looking For

• Active, unencumbered LPN or RN license in Arizona

• Current CPR certification

• At least 1 year of experience in home care, senior care, or a related setting

• Excellent communication and teaching skills

• Compassionate, dependable, and team-oriented

• Valid driver’s license and reliable transportation

• Ability to pass a background check

Compensation & Benefits

• $58,000 – $72,000 per year, based on experience and license

What We’re Looking For
