Responsibilities

The RN's practice is guided by the ANA Standard for Professional Nurse and Code of Ethics. The RN has knowledge of professional nursing principles, practices, concepts, and procedures as applied to the care of the patient population served. The RN records patients' medical history and symptoms, helps perform diagnostic tests and analyze results, operates medical equipment, administers treatment and medications, and helps with patient follow-up and rehabilitation.

The RN establishes or contributes to a care plan that may include activities such as administering medication, checking dosages, managing intravenous lines for fluid, medication, blood, and blood products; administering therapies and treatments; observing the patient and recording observations; and consulting with physicians and other healthcare clinicians. The RN teaches patients and their families how to manage illnesses or injuries, explains post‑treatment and home‑care needs, diet, nutrition, and exercise programs, and self‑administration of medication and therapy.

The RN provides direction to licensed practical nurses and nursing aides regarding patient care and delegates when appropriate. The RN supports efforts to advance clinical knowledge and skills, precepts new hire staff and students, assists them in completing competency skills, and enhances knowledge in nursing management and patient care as required. The RN may continue to improve patient experience, clinical outcomes, and promote a healthy work environment.

Nursing practices are theory‑based, evidence‑based, and utilize a shared governance model for decision making. All RN staff who have direct responsibility for the care of stroke patients must have knowledge of:

  • Stroke symptoms and warning signs
  • Protocols and pathways used to identify, evaluate, and manage stroke patients
  • NIHSS
  • Stroke Alert activation and protocol
  • Stroke quality indicators
  • Available stroke patient education materials and community resources
Essential Functions
  • Collect health data systematically, prioritizing collection based on patient needs and involving family; analyze assessment data to determine diagnoses and develop, implement, evaluate, and revise family‑centered, developmentally and culturally relevant care plans.
  • Identify individualized expected outcomes and develop plans of care reflecting evidence‑based practice, ensuring continuity of care.
  • Implement plan‑of‑care interventions, initiate treatments, medications, emergency and resuscitative measures based on standing orders, policies, and procedures; provide nursing care that meets physical, emotional, spiritual, and social‑cultural needs using a family‑centered approach.
  • Evaluate patient progress toward outcomes, document observations, interventions, therapeutic measures, and monitoring data in a retrievable form; continuously assess quality and effectiveness of the care plan.
  • Evaluate own nursing practice relative to professional practice standards, statutes, regulations, and department policies; maintain current knowledge and systematically evaluate quality of nursing practice.
  • Integrate ethical principles in all practice aspects and enhance knowledge of cultural and diversity needs for specific populations.
  • Collaborate with peers and other health care providers on professional development through role modeling, fostering peer relationships, and participating in preceptor and mentor programs to create a quality practice environment.
  • Collaborate with multidisciplinary team members to assess, plan, implement, and evaluate patient care delivery; involve the patient, family, and providers in care.
  • Develop and evaluate evidence‑based standards of care, identify changes needed in nursing practice, facilitate adoption and adaptation of changes, and support research integration into practice.
  • Consider safety, effectiveness, and cost factors in planning and delivering patient care, utilizing resources efficiently.
