Our structure provides Home Health Nurses multiple avenues for support not available at other agencies – a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available daily), an in‑office team of QI specialists, telehealth resources, a clinical manager and in‑office triage support.

As a Home Health Registered Nurse, you will join the Banner Home Health Clinical Team providing exceptional, expert care in your community. You will enjoy an autonomous practice focusing on medical intervention within the patient’s home environment and the ability to create one‑on‑one relationships with your patients and their caregivers. You will have flexibility over your schedule that is not possible in the hospital setting, and you will work in a sub‑assigned geographic territory with a smaller travel distance than other valley agencies.
You will receive mileage reimbursement and company equipment (cell phone and tablet for documentation in our EMR system (Home Care Home Base)).
This position offers a flexible schedule – 5 8‑hr shifts or 4 10‑hr shifts, with weekend rotation every 4‑5 weeks. 1:1 home care in the East Valley covering Tolleson, Laveen, Phoenix, South Glendale, Avondale, Goodyear, Buckeye, Tonopah. Flat rate $3/hour weekend shift differential.
Core Functions- Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
- Reviews patient history and physical with patient/family; assures completion within appropriate timeframe; reviews information from other health care team members, diagnostics, laboratory data and reports abnormal results.
- Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input.
- Initiates and implements the plan of care based on patient‑specific needs; develops realistic, measurable goals; collaborates with multidisciplinary team; revises plan as needed.
- Delegates and supervises health care team activities; promotes continuity of care via accurate communication.
- Evaluates patient and family response to care and teaching; revises care plan as needed; evaluates progress toward goals.
- Documents assessment, planning, implementation and evaluation in the patient record; ensures legible, timely documentation per policy.
- Provides evidence‑based care; participates in research activities, unit leadership, education and orientation of personnel, patients, families and visitors.
- Contributes to society through activities that lead to excellent patient outcomes; participates in professional organizations; improves national nursing quality indicators and outcomes.
- May supervise staff and workflow of the department in some roles.
- Interacts with all levels of staff and external contacts; provides spiritual support; synthesizes and prioritizes data from multiple sources.
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification required. Additional certification or continuing education may be required based on area of practice. Experience in the clinical area for which the applicant is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work.
Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Home Health team members must operate a personal vehicle and possess a valid driver’s license and active auto insurance. Required background clearance cards depend on the specific behavioral health unit and state; please refer to the original requirement for detail.
Bachelor’s degree preferred. Professional certification preferred.
