LPN Intake Referral Coordinator

In this fast-paced office role, you'll serve as the primary liaison between hospitals, physicians, referral sources, patients, families, and our clinical teams to ensure a smooth and timely admission process. If you're organized, detail-oriented, and enjoy coordinating care, we'd love to meet you!
What You'll Do- Receive and process new patient referrals accurately and efficiently.
- Verify insurance benefits, payer sources, and obtain required authorizations.
- Enter and maintain patient information within the Electronic Medical Record (EMR).
- Coordinate admissions with Nursing Supervisors and interdisciplinary clinical teams.
- Communicate professionally with referral sources, physicians, patients, families, and staff.
- Ensure compliance with agency policies, Medicare guidelines, and documentation standards.
- Participate in quality improvement (QAPI) initiatives and ongoing education.
- Current Texas LVN/LPN License
- Minimum one year of referral intake experience preferred
- Experience with Medicare, Medicaid, and third-party insurance verification preferred
- Medical office or healthcare background with knowledge of medical terminology
- Strong computer, organizational, and customer service skills
- Home Care Home Base experience preferred
- Valid Driver's License and Auto Insurance
