As an Advanced Practice Provider - NP, you play a pivotal role in delivering high-quality patient care. You serve as a primary healthcare provider, blending clinical expertise with a compassionate approach to improve patient outcomes. This role empowers you to make significant contributions to our patients' health journeys, working autonomously while collaborating closely with a dedicated healthcare team. You directly impact patient well-being by providing comprehensive, evidence-based care in a dynamic clinical environment.

Job Duties and Responsibilities
  • Conducts comprehensive patient assessments, gathering detailed medical histories and evaluating current health status.
  • Formulates accurate diagnoses based on clinical findings, laboratory results, and diagnostic imaging.
  • Develops individualized treatment plans, integrating patient preferences and evidence-based guidelines.
  • Prescribes appropriate medications and other therapeutic interventions, monitoring efficacy and managing side effects.
  • Performs thorough physical examinations to identify health issues and monitor patient progress.
  • Maintains accurate and timely patient records within Electronic Health Records (EHR) systems, ensuring data integrity and accessibility.
  • Educates patients and their families on health conditions, treatment options, preventive care, and self-management strategies.
  • Communicates effectively with patients, families, and interdisciplinary healthcare team members to ensure coordinated care.
  • Applies critical thinking to complex clinical situations, making sound decisions under pressure.
  • Manages chronic diseases, developing long-term care strategies and promoting patient adherence to treatment plans.
Required Qualifications Education
  • Holds a Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
  • Possesses current, unrestricted Nurse Practitioner licensure in the state of practice.
  • Maintains current national board certification as a Nurse Practitioner.
Experience
  • Demonstrates a minimum of two years of clinical experience as a practicing Nurse Practitioner.
Knowledge/Skills
  • Expertly assesses patient health status through comprehensive evaluation.
  • Accurately diagnoses a wide range of medical conditions.
  • Develops effective and personalized treatment strategies.
  • Safely prescribes and manages various medications.
  • Proficiently performs physical examinations across diverse patient populations.
  • Demonstrates proficiency in using Electronic Health Records (EHR) systems for documentation and patient management.
  • Effectively educates patients and families on health management and disease prevention.
  • Communicates clearly, empathetically, and professionally with all stakeholders.
  • Applies strong critical thinking and problem-solving skills to complex clinical scenarios.
  • Manages complex chronic diseases with a focus on long-term patient health.
  • Collaborates effectively within a multidisciplinary team.
Preferred Qualifications
  • Holds additional certifications in a relevant specialty area (e.g., Family, Adult-Gerontology, Pediatrics).
  • Possesses experience in a primary care or urgent care setting.
Physical Requirements /Work Conditions
  • Ability to stand, walk, and sit for extended periods.
  • Capable of lifting and positioning patients as needed.
  • Possesses manual dexterity to operate medical equipment and perform examinations.
  • Works in a clinical environment with potential exposure to infectious diseases and medical waste.
  • Maintains visual and auditory acuity for patient assessment and communication.
