Care Plus Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to join our Phoenix-area hospice team. You will coordinate, educate, and implement individualized care plans for an assigned patient caseload, providing skilled nursing visits in homes and facilities.
Work with the interdisciplinary team to ensure high-quality end-of-life care, documentation, and on-call coverage. Arizona RN license, CPR, and 1+ year nursing experience required.
