LPN, LPN​/LVN, Nursing Home

Human Good community is seeking a compassionate LPN to provide high-quality nursing care to older adults in our senior living community. In this role, you will administer medications, monitor resident health, support daily living needs, and collaborate with an interdisciplinary team to deliver person-centered care. You'll maintain accurate clinical documentation, follow infection control and safety standards, and respond to changes in condition.

Responsibilities

• Provide direct nursing care and medication administration to residents in accordance with care plans and state regulations
• Monitor, document, and report residents' health status, changes, and responses to treatment to the interdisciplinary team
• Support residents with daily living activities while promoting dignity, safety, and independence
• Collaborate with RNs, physicians, and care team members to deliver high-quality, person-centered care
• Educate residents and families on care plans, treatments, and wellness practices
• Maintain accurate electronic health records and ensure compliance with policies and regulatory standards
• Respond to emergencies, perform basic clinical procedures, and follow infection control protocols
• Participate in quality improvement initiatives, training, and team meetings to enhance resident outcomes
