Field Nurse Practitioner / Physician Assistant

You’ll have the opportunity to bring high-quality, compassionate care directly to patients in their homes and communities—meeting them where they are and making a meaningful difference in their everyday lives. You'll serve as the primary care provider for Medicare and Medicaid patients, helping improve health outcomes through in‑home visits, care coordination, and chronic disease management.

Openings in Southeast Phoenix and East Valley, AZ (Tempe, Southeast Scottsdale, and Central East Phoenix)

• $120,000 - $142,000 annually (DOE)
• Eligible for up to 7.5% Annual Bonus
• Mileage Reimbursement (IRS rate for travel over 30 miles)

• Monday – Friday | 8am-5pm
• No weekends

• Conduct 5 to 7 daily in-home and occasional telehealth visits, performing comprehensive patient assessments, diagnosing conditions, prescribing treatments, and managing ongoing care plans
• Educate patients on chronic disease management, coordinate care with interdisciplinary teams, and maintain accurate documentation in Athena EMR

• Active Arizona NP or PA license
• 1+ year of primary care, family practice, or outpatient experience
  
  Active DEA license
• Current BLS certification
