Outpatient Stroke APP - Scottsdale, Arizona

Honor Health Medical Group is seeking a full-time Nurse Practitioner or Physician Assistant to join its growing outpatient stroke program at the Honor Health Neuroscience Institute at Osborn
.

The APP will work alongside two experienced stroke neurologists and primarily care for patients requiring ongoing management after stroke, hospitalization or other cerebrovascular conditions.

• Full-time outpatient position
• Located at the Honor Health Neuroscience Institute at Osborn
• Primarily follow-up and post-discharge care
• Dedicated medical assistant support
• Collaborative stroke and neuroscience team
• Epic EMR
• No inpatient rounding or call requirements

• Evaluate and manage patients following stroke or hospitalization
• Provide timely post-discharge follow-up care
• Care for patients with stroke, TIA, carotid artery disease, cerebral blockages and a history of hemorrhage
• Review imaging, laboratory results and hospital records
• Develop treatment and secondary stroke-prevention plans
• Educate patients and families and coordinate care with the stroke team

• Certified Nurse Practitioner or Physician Assistant
• Current or eligible Arizona license
• Neurology, stroke or related experience preferred
• Strong clinical judgment and a collaborative approach to care
