Client Relationship Manager
Traditional Home Care has an immediate opening for a Care Coordinator in Montrose, Pa.
Due to expansion and significant growth, our Non‑Medical Division seeks an aggressive candidate for this key opportunity. The position is salaried, paid weekly, and includes an aggressive bonus plan along with a generous benefits package.
As the Care Coordinator
, you will monitor and support the daily activities of services provided to clients in accordance with state regulations and company policy, ensuring quality coordination of service for our clients.
- Provide superior customer service and quality home care
- Build lasting relationships with clients, referral sources, payers, and community organizations
- Coordinate client services and manage scheduling issues
- Manage and grow your client caseload
- Develop strong, communicative relationships with the team
- Interview, hire, and orient reliable and compassionate caregivers
- Function as primary support for daily EVV and payroll functions
- Previous home health care or home care experience highly desirable
- High School or GED required;
Associate’s Degree preferred - Staffing experience preferred
- Excellent verbal/written communication, time management and organizational skills
- Ability to effectively multi‑task
- Effective problem solving skills
- Strong presentation and training skills
