Health Care Aide

Position Summary

The Health Care Aide provides direct client care and environmental support within established standards of care, policies and procedures. The Health Care Aide functions in meeting the special and changing needs of clients and assists them in reaching and maintaining optimum health and independence under the direction of the team leader.

• Assist clients in activities of daily living, maintaining self-respect, dignity, and independence; integrate the resident Bill of Rights into care.
• Follow individualized clients’ care plans, provide feedback to improve care, and communicate/report client changes to the team leader.
• Document care and observations in the client’s health record and appropriate flow sheets.
• Assist clients with personal hygiene, elimination, mobility, and nutrition per care plans.
• Provide or assist with medical equipment use, vital signs, oxygen therapy, suction systems, and medication safety principles.
• Maintain a clean and safe environment, including equipment storage, infection control, and hand hygiene.
• Participate in admissions, transfers, discharges, and in care of the body after death.
• Promote client participation with activity programs and facilitate leisure activities.
