Responsible for gathering essential intake information from acute care patients and their families, while also helping with discharge planning. Collaborates closely with other healthcare providers, social services, and community resources to address patient needs, serving as a patient advocate to ensure they make use of all available services.
- Requires an Associates Degree in Social/Human Services or related field or equivalent combination of education/experience;
- Excellent written/oral communication skills, and interpersonal/telephone skills;
- Ability to manage multiple/changing priorities.
Constantly: (Exists 75% or more of the time) Walking; lifting and carrying 0-5 lbs.
Frequently: (Exists 25-75% of the time) Standing; sitting w/back support; operating machinery; exposure to radiation.
Occasionally: (Exists 5- 25% of the time)
Reaching (overhead, forward, lateral, low); climbing; twisting; lifting and carrying 5-25 lbs.; pushing and pulling 5-15 lbs.
Rarely: (Exists under 5% of the time) Stooping (bend at waist); crouching (bend at knees); kneeling; balance; pushing and pulling 15-40 lbs.; exposure to chemical hazards, infect hazards, slippery surfaces.
Sensory Requirements Visual: close paperwork, VDT used;
Hearing:conversation, telephone, background noise.
