Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform In‑Home Health Evaluations. Visits last about 45 minutes with medication/medical history review and physical exams; diagnostic tests may be ordered by the health plan.
An iPad and workflow guide the evaluation; prescriptions or treatment changes are not required. Role requires flexibility to travel locally within licensed states and to conduct virtual visits when needed.
