** About One Well Health Care
** One Well Health Care is a U.S.
-based healthcare provider dedicated to serving individuals with Intellectual and Developmental Disabilities (I/DD) and Autism. With compassion, professionalism, and purpose, we support every step of our participants’ journeys toward independence and well-being.
** Position Summary
** The Behavior Support Specialist (BSS) Hybrid role provides specialized interventions to assist participants in increasing adaptive behaviors that replace or modify challenging behaviors of a disruptive or destructive nature. These behaviors often interfere with the participant’s inclusion in home, family, or community life. The role promotes consistent implementation of the Behavioral Support Plan (BSP) and Crisis Intervention Plan (CIP) across environments and individuals who interact regularly with the participant—such as family members, friends, neighbors, and other providers.
Consistency is essential for skill development and the reduction of problematic behavior.
This position includes responsibilities related to the initial BSP development as well as ongoing behavioral support. Additional responsibilities may include, but are not limited to: conducting Functional Behavior Assessments (FBA), writing Behavior Support and Crisis Intervention Plans, attending annual ISP meetings, preparing quarterly reports, coordinating with support coordinators, conducting meet-and-greets as needed, and participating in weekly one-on-one supervision meetings with the immediate supervisor.
** Ongoing Support
** Ongoing support may occur both before and after the completion of the BSP. If a participant requires behavioral support prior to the BSP and CIP development, the Support Coordinator (SC) may submit a request to the Office of Developmental Programs (ODP) for ongoing support during plan development. This service may be provided in the participant’s home or other community locations.
** Direct Support includes:**- Providing consultation and support to the participant to help them understand the BSP’s purpose, objectives, methods, and documentation; evaluate its effectiveness; and review recommended revisions.
- Attending meetings with colleagues to meet potential participants and discuss desired behavioral outcomes.
- Delivering crisis intervention support directly to the participant during acute behavioral episodes that pose imminent risk of harm to self or others, potential institutionalization, incarceration, or significant property damage.
** Consultative Support includes:**- Supporting family members, friends, waiver providers, and employers to understand the BSP’s purpose, objectives, and implementation methods, including data collection, documentation, and revisions agreed upon with the participant.
- Monitoring and analyzing data collected during BSP implementation to evaluate progress toward established goals.
