Social Worker
- Home Hospice

Full-Time

Day Shift (8am - 5pm)

This role works in collaboration with the Interdisciplinary Team to provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers.
MSW required, LSW or LCSW is also required. It is the expectation that license is obtained within 2 years of hire. Two years’ experience in the health care setting preferred.

* Must work a minimum of 64 hours per two-week pay period
