Job Duties:
  • Evaluate residents and develop individualized treatment plans.

  • Provide evidence-based physical therapy services to improve mobility, strength, balance, endurance, and functional independence.

  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families to achieve the best outcomes.

  • Educate residents and caregivers on treatment plans, exercises, and injury prevention.

  • Accurately document evaluations, treatment sessions, progress notes, and discharge summaries.

  • Participate in interdisciplinary care planning and quality improvement initiatives.

  • Help residents safely return to their highest level of function and independence.

Qualifications
  • Current Physical Therapist license in the applicable state.

  • Graduate of an accredited Physical Therapy program.

  • Strong clinical, communication, and interpersonal skills.

  • Passion for providing exceptional resident-centered care.

  • Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
