Deer Meadows Rehabilitation Center is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Philadelphia, Pennsylvania

Our therapists work closely with nursing, physicians, and interdisciplinary staff to provide outstanding, patient-centered care in a supportive Skilled Nursing and Rehabilitation environment.
Position ResponsibilitiesEvaluate residents and develop individualized physical therapy treatment plans.
Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.
Establish functional goals that promote mobility, strength, balance, endurance, and independence.
Collaborate with physicians, nursing staff, and the interdisciplinary team to ensure optimal patient outcomes.
Educate residents and families regarding treatment plans, mobility techniques, and discharge recommendations.
Complete accurate and timely documentation using Net Health and Point Click Care (PCC).
Maintain compliance with all federal, state, and company regulations.
Current Physical Therapist (PT) license in the Commonwealth of Pennsylvania.
Graduate of an accredited Physical Therapy program.
Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
Experience with
Net Health and Point Click Care (PCC) is a plus.Strong communication, clinical reasoning, and organizational skills.
Ability to work collaboratively in a fast-paced rehabilitation environment.
