An opportunity has arisen to join a highly regarded andestablishedteam based at Hinchingbrooke Hospital, at a time of growth and change.
Wewillrecruit an enthusiastic and motivated
Clinical Nurse Specialistor Social Workerto join our Psychiatric Liaison Team.
We are particularly keen to hear from applicants with experience of working across age ranges.
- To assist in the delivery of a high-quality Liaison Psychiatry Service within Hinchingbrooke Hospital in keeping with the visions and values of the Cambridgeshire and Peterborough NHS Foundation Trust.
- Providing and undertaking timely psychosocial assessments and risk management plans in accordance with CPFT’s Clinical Risk Assessment and Management policy for adults presenting to the emergency department with mental health problems.
- Promoting a safe and effective service throughout ED; this will be through training and giving expert advice and support to nursing and medical teams in ED.
- Maintaining a visible, professional presence where patients, carers and staff can turn for assistance, advice and support.
- As part of the Liaison Psychiatry Service, the post-holder will contribute to the multidisciplinary clinical assessment and management of patients referred to the service in collaboration with other team members.
- To participate in and promote close links with community mental health teams and other agencies to develop effective care plans.
