Aging Life Care Manager

We are looking for licensed social workers, RN’s, OT’s or other licensed medical professionals seeking a fulfilling, relationship-driven role outside of a traditional clinical setting.

Complete Care Strategies, a well-established Aging Life Care Management practice serving the greater Philadelphia suburbs for over 26 years, is looking for a compassionate individual to join our highly supportive and collaborative team.

In this role, you will:

• Conduct client assessments and develop care plans
• Visit clients and support ongoing care needs
• Communicate regularly with families and providers
• Provide guidance during times of transition or change

We’re looking for someone who:

• Has at least 5 years experience with older adults, special needs clients, and the medically complex
• Is highly organized and comfortable managing multiple moving parts
• Is comfortable using technology and thinking outside of the box
• Buil ds strong relationships and communicates thoughtfully
• Demonstrates emotional intelligence and healthy boundaries

Requirements:

• PA Licensure in your particular discipline
• CCM or CMC certification a plus
• Ability to travel for in-person client visits in the Greater Philadelphia area
