PRN Occupational Therapist (OT) – Home Health

Deer Meadows Home Health & Support Services is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our growing home health team. This is an excellent opportunity for an OT who enjoys working independently while making a meaningful impact by helping patients achieve greater independence in the comfort of their own homes.
ResponsibilitiesEvaluate patients and develop individualized occupational therapy treatment plans.
Provide skilled occupational therapy services in the home setting.
Assess functional abilities, home safety, adaptive equipment, and activities of daily living (ADLs).
Educate patients, families, and caregivers on treatment plans and home safety recommendations.
Collaborate with physicians, nurses, and the interdisciplinary care team to ensure optimal patient outcomes.
Complete accurate, timely documentation using Homebase Home Care EMR
.Maintain compliance with agency policies, Medicare guidelines, and state and federal regulations.
Current Occupational Therapist (OT) license in PA.
Graduate of an accredited Occupational Therapy program.
Home health experience preferred, but not required.
Experience with
Homebase Home Care EMR is a plus.Strong clinical assessment, communication, and organizational skills.
Current CPR certification (or ability to obtain).
Valid driver's license, reliable transportation, and current auto insurance.
