Job Summary:
We are seeking a dynamic and experienced Social Services Director to join our team at the Dubois Continuum of Care Community in DuBois, Pennsylvania. The ideal candidate will possess a strong background in senior care or community-based organizations, with a passion for delivering exceptional social services to our residents. As a key member of our team, you will be responsible for planning, organizing, developing, administering, and evaluating our social services program, as well as overseeing the operation of the department and staff.

As a key member of our team, you will be responsible for planning, organizing, developing, administering, and evaluating our social services program, as well as overseeing the operation of the department and staff.
Job Responsibilities:- Supervise Social Services staff, including performance evaluations and all personnel-related matters
- Hold individual and department staff meetings on a regular basis
- Develop and maintain policies and procedures for the Social Service department, ensuring compliance with regulations
- Facilitate various internal programs that enhance residents' psycho-social functioning, such as Resident Council, QAPI teams, family groups, and other committees as needed
- Practice supportive intervention, counseling with residents to resolve conflicts, and provide education and guidance
- Serve as the liaison to hospice providers and complete MDS assessments, documentation, and care planning as required
- Participate in interdisciplinary team meetings, providing professional feedback and education as requested
- Prepare and maintain the department budget and monitor departmental expenditures
- Complete daily, weekly, monthly, quarterly, and annual reporting requirements
- Maintain a safe work environment and adhere to facility policies and procedures
- EXPERIENCE:
Preferred 2-year experience in a nursing home or assisted living/personal care setting, with minimum of 2 years in management supervisory role - EDUCATION:
High school diploma or GED required, Bachelor's degree in social work or related field required, LSW or MSW preferred
