Position Overview
At All American Hospice, we deliver end-of-life care with precision, dignity, and compassion. Our hospice nurses are trusted clinical leaders that provide advanced pain and symptom management while supporting patients and families through one of life’s most critical moments.
Responsibilities- Deliver comprehensive hospice nursing care in accordance with physician orders and hospice standards
- Perform patient assessments and develop, implement, and update individualized plans of care
- Manage pain and complex symptoms related to terminal illness
- Ensure accurate, timely documentation that meets Medicare and reimbursement guidelines
- Coordinate patient care with caregivers, physicians, and interdisciplinary team members
- Communicate effectively with patients and families, building trust and rapport at all levels
- Participate in Interdisciplinary Team (IDT) meetings and quality reviews
- Provide on-call coverage a minimum of one week per month, including evenings, weekends, and holidays as assigned
- Travel as required based on agency needs
- Active Registered Nurse (RN) license in Pennsylvania
- Minimum 2–3 years of clinical nursing experience
- Hospice or palliative care experience strongly preferred
- Strong clinical assessment and critical-thinking skills
- Excellent communication with patients, families, and care teams
- Valid driver’s license and reliable transportation
- Experience with Medicare hospice documentation
- Home health or hospice case management background
- High level of clinical autonomy
