Immediate opening for a Registered Nurse RN Case Manager Home Health in Lackawanna County, PA including Scranton, Dunmore, Taylor, and Moosic. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.

PAY AND BENEFITS

* $38-$40 per hour
* $5,000 sign-on bonus

* Weekly pay with direct deposit

SCHEDULE

* Monday through Friday 8:30 AM to 5:00 PM

* On-call, weekend, and holiday rotation

REGISTERED NURSE RN CASE MANAGER RESPONSIBILITIES

* Manage a home health caseload in Lackawanna County

* Perform RN assessments and reassessments

* Develop and update care plans

* Coordinate care with physicians and healthcare team

* Complete home health documentation and ensure compliance

* Educate patients and families

REQUIREMENTS

* Active Pennsylvania Registered Nurse RN license

* Minimum 1 year home health RN experience

* Case Manager or Case Management experience preferred

* CPR certification

* Valid driver's license and reliable transportation
