Job Summary

At Geisinger, professionals grow through quality improvement and interdisciplinary collaboration. This liaison position primarily covers Geisinger Wyoming Valley (GWV), Geisinger South Wilkes-Barre (GSWB), and Geisinger Community Medical Center (GCMC). Responsibilities include in‑person support across these sites, coordinating patient discharges from hospital to home infusion settings, and supporting ongoing patient care. A typical schedule is Monday through Friday 8:30AM to 5:00PM, with rotating weekends and holidays.

Responsibilities
  • Coordinate and deliver comprehensive infusion therapy services in home, suite, and other clinical settings.
  • Manage referrals, verify clinical and administrative requirements, and provide direct patient care, education, and case management.
  • Collaborate with physicians, pharmacists, and interdisciplinary teams for safe, effective, patient‑centered care.
  • Coordinate intake and referral processes, verifying demographics, clinical history, authorizations, and consents.
  • Develop and implement individualized care plans based on thorough patient assessments, including venous system evaluation.
  • Prepare and administer infusion medications using aseptic technique; perform vascular access procedures and lab draws.
  • Educate patients and caregivers on therapy protocols, equipment use, and vascular access care.
  • Monitor therapy progress, adjust care plans, and communicate with providers to ensure optimal outcomes.
  • Coordinate care and communicate effectively with providers, pharmacists, internal departments, and external health‑care teams for continuity and quality care.
  • Maintain accurate documentation in the EMR and ensure compliance with clinical protocols and accrediting standards.
  • Respond to patient and caregiver concerns, escalating issues appropriately for resolution.
  • Participate in the development of infusion protocols, patient education materials, and quality improvement initiatives.
  • Support 24/7 care coverage through on‑call rotations, including weekends and holidays.
  • Collect and analyze clinical data to inform care decisions and contribute to the evaluation of advanced care models.
  • Travel to patient homes and clinical sites to deliver infusion therapy, provide infusion‑related patient care, and perform patient teaching and assessments.
  • Provide clinical oversight and guidance to Licensed Practical Nurses, ensuring adherence to care standards and supporting professional development.
  • Perform other duties as assigned.
Qualifications
  • At least 3 years of registered nursing (RN) experience.
  • RN Home Health and IV therapy experience preferred.
  • Graduate from a Specialty Training Program in Nursing (Required).
  • Basic Life Support Certification (Required).
  • Licensed Registered Nurse (Pennsylvania) – RN State of Pennsylvania (Required).
  • Clinical skills: patient assessment and care, venous access, infusion medication preparation.
  • Communication and interpersonal skills for interdisciplinary collaboration.
  • Computer literacy and health information documentation proficiency.
