The Dialysis Home Therapies Nurse coordinates the administrative and clinical aspects of our home dialysis program, supporting both home hemodialysis and peritoneal dialysis treatment options.
Please note:
This is not a traditional home health position requiring daily travel or field-based home visits. Instead, this role is centered within our clinic, focusing heavily on clinical coordination, patient education, and interdisciplinary collaboration to empower patients to manage their care safely will act as the organizational anchor for patients learning to navigate their home hemodialysis or peritoneal dialysis journeys.
Schedule: Fulltime, Monday through Friday, shifts starting at 7am; on call as needed
Compensation: Pay range from $35-$44 per hour, depending on nursing and dialysis experience. A $8,000 sign on bonus is available!

- Collaborate with medical team on patient care concerns, hospitalizations, discharges and/or modality changes, etc.
- Coordinate home training sessions, clinic visits, patient care conferences, home visits and all clinical and educational activities to assure efficient and productive use of nursing hours and program facilities
- Monitor patient supply ordering and usage while maintaining quality care
- Communicate regularly regarding home program activities with interdisciplinary care team
- Monitor ongoing patient care parameters
- Perform all registered nurse functions and duties
- Create and maintain appropriate record keeping, patient training and documentation in accordance with ESRD Network, regulatory agencies, DCI CQI program and the dialysis facility's policies and procedures
- Excellent communication skills and a desire to teach
- Demonstrated clinical excellence

Education/Training:
- Accredited nursing degree
- Dialysis experience a plus
- Current PA RN license
- Valid driver's license and safe driving record required
