Care Givers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one‑on‑one pediatric care in the home setting throughout parts of East Lackawanna, Wayne, Wyoming, and Susquehanna counties.
This is a salaried Monday–Friday role with a company vehicle provided.

You will manage care for pediatric patients in their homes, serving as the clinical lead and trusted partner to families.

Territories to cover:
Luzerne, Wyoming, and Sullivancounties
Schedule:
Full‑Time | 40 hours per week (traveling included) | Monday–Friday

- Complete comprehensive patient assessments, including OASIS data collection, within established timelines
- Develop and oversee individualized pediatric care plans in collaboration with physicians and families
- Provide clinical direction and supervision to LPNs and Home Health Aides
- Ensure documentation accuracy, timeliness, and regulatory compliance
- Communicate consistently with families and interdisciplinary care teams
- Participate in quality improvement initiatives and staff education
- Maintain a safe, respectful, and supportive care environment
- Active Pennsylvania RN license (Required) – BSN preferred
- 1 year of professional nursing experience preferred
- Pediatric or home health experience preferred
- Strong leadership and communication skills
- Ability to work independently and manage multiple priorities
- Valid driver’s license
- Current CPR certification
