Position: County Caseworker 1 (Local Government) - Wayne County MH/ID

THE POSITION

Located in Honesdale, PA, Wayne County Behavioral Health & Intellectual Disabilities agency is looking for you. The Blended Caseworker is responsible for planning, coordinating, and managing services for assigned caseloads, utilizing recovery-oriented and trauma-informed practices. DESCRIPTION OF WORK

This position supports children, adolescents, and their families by guiding them through needed services and community supports. As a County Caseworker 1, you will perform the following duties:

  • Intake Support:
    Gather information during initial intake to help enroll individuals in the Behavioral Health Blended Case Management Program
  • Service Coordination:
    Work with families, providers, and the  Coordinator to link individuals with needed community services
  • Needs Assessment:
    Use the Environmental Matrix to assess level of service needs on an ongoing basis
  • Recovery Guidance:
    Promote resiliency and recovery by following Behavioral Health principles that support everyday living
  • Specialized Assistance:
    Apply specialized training to support individuals with mental illness and co-occurring intellectual disabilities or autism spectrum disorders
  • Progress Monitoring:
    Maintain close contact with families and providers to evaluate services and track progress toward treatment goals
Work Schedule and Additional Information

Full-time employment; 37.5 hours/week. Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch.

Employee may have overnight travel or out of County travel for meetings or training. Employee may have irregular hours, including evenings, weekends. Employee works on-call on a rotating basis 24/7 once trained.

Required Experience, Training & Eligibility QUALIFICATIONS
  • Two years of experience as a County Social Services Aide 3 and two years of college level coursework which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
  • A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
Other Requirements
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.

  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
