Position: Caseworker 3 - AE

This position is responsible for complex case planning and team collaboration assistance for individuals registered to the York/Adams Mental Health Intellectual and Developmental Disabilities Program Administrative Entity. Additional responsibilities include assisting with Quality Assessment and Improvement (QA&I) processes required by the Office of Developmental Programs.

  • Assess the needs of individuals requiring urgent service needs or changes, assist supports coordination team to prepare care plans, authorize service, and reassess the need for continuation of service on an on-going basis.
  • Work closely with and at direction of Quality Management supervisor to compile data for review and analysis by team partners.
  • Work in close cooperation with ODP licensed providers, other social agencies, hospitals, clinics, courts, and other community resources in the course of navigating care for individuals with extremely complex needs.
  • Prepare case records, progress notes, write letters, make reports, and maintain records.
  • Participate in inter-disciplinary conferences with other agency staff persons.

Provide information and training to other /A service professionals and lay persons regarding QA&I activities and processes.

  • Obtain information from consumers, family members, medical professionals and others as needed in order to identify social, economic, emotional, health and physical problems and recommend appropriate resolutions.
  • Maintain a working knowledge of medical terminology, therapies and medications.
  • Become an authority on ODP service definitions.
  • First point of AE review for variance requests and tracking of variances for the program.
  • Maintain a working knowledge of community resources to be able to include available resources as a factor in assessing the consumer’s circumstances and making recommendations in the resolution of incidents.
  • Enter data as required into computer on a timely basis.
  • Participate in relevant training and meetings.
  • Perform other duties as assigned.
  • Experience & Training:
    • A bachelor’s degree in the field of Human Services (psychology, sociology, behavioral sciences, social work, etc.), criminal justice or other related social sciences and three years of experience in public or private social work, including one year of direct /A service experience; or
    • Two years as an Intellectual Disabilities / Autism (/A) Caseworker 2; or
    • Any equivalent combination of experience and training which must include successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • Knowledge of the laws, rules and regulations governing the /A program system.
  • Knowledge of ODP guided QA&I activities and Quality Management Certification (or ability to obtain within 90 days of hire or transfer).
  • Knowledge of social casework principles and methods.
  • Knowledge of the availability of community support services for the benefit of individuals eligible for ODP services.
  • Knowledge of current social, economic and health problems and resources as they relate to individuals with a diagnosis of intellectual disability or autism and their natural support system.
  • Knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional or health problems.
  • Skill in the development of plans to address the needs of individuals participating in ODP services.
  • Ability to establish and maintain effective working relationships with clients, other members of the staff, outside agencies, institutions and the general public.
  • Ability to plan and organize work, prepare adequate records and reports, set priorities and complete the QA&I process by all deadlines.
  • Ability to clearly express ideas orally and in writing and to interpret laws and regulations.
  • Ability to maintain confidentiality in high pressure situations.
  • Ability to understand and accept the needs and rights of other persons.
  • Ability to work effectively with people and aid them to grow in the constructive use of their potential in adjusting to their specific problems.
