Families United Network, a leading biotechnology firm improving health outcomes, seeks a Foster Care Case Manager to support children and foster families through evidence-informed, outcomes-focused case management. In this role, you will manage a caseload of foster youth, conduct intakes and home visits, develop and monitor care and treatment plans, and coordinate services with healthcare, education, and community providers. You will maintain accurate electronic documentation, advocate in court and team meetings, and apply trauma-informed practices.

Responsibilities
- Manage a caseload of foster children and families, ensuring safety, stability, and compliance with agency and regulatory standards.
- Conduct intakes, home visits, and regular assessments to evaluate child well-being and placement suitability.
- Develop, implement, and monitor individualized care and treatment plans using evidence-informed practices.
- Coordinate services with healthcare providers, schools, therapists, and community agencies to address child and family needs.
- Maintain timely, accurate electronic case records, reports, and documentation for audits and court requirements.
- Participate in court hearings, team meetings, and case reviews, advocating for the best interests of each child.
- Apply trauma-informed and culturally responsive approaches in all interactions with children and caregivers.
- Provide crisis intervention, de-escalation, and safety planning as needed to support placement stability.
- Collaborate closely with internal multidisciplinary teams within a biotech-informed, outcomes-focused environment.
- Educate and support foster parents on care plans, resources, and best practices to improve long-term outcomes.
