The Complex Care and Review team is an effective social care team consisting of Social Workers, Social Care Assessors, Occupational Therapy staff and a Specialist Accommodation Team.

This is an experienced team of highly motivated, hardworking, and skilled professionals. We are looking for a candidate who demonstrate these skills and have a passion to drive forward positive outcomes for individuals regardless of their level of need or stage of life they are in.

Candidates will be required to work collaboratively and creative with colleagues, and to have an in-depth knowledge of the Care Act 2014 and a sound knowledge of current legislation, policy, and procedures within adult services. We are looking for a Qualified Social Worker to join the day service element of the team. We are embedding the Progression model and seeking community-based opportunities for those who are ready to move on from traditional day services to achieve their personal aspirations.

We are working closely with day services, voluntary sector, the supported employment team and the ASC employment and progression team to support people we support with learning disabilities.

There are opportunities to work in other areas of the team in the future. The remit also includes project work, working closely with operational teams and commissioners in the future forecasting of service-user’s needs within CW&C, identifying gaps in services and steering new bespoke commissioning opportunities. The team is unique in that it completes reviews taking a ‘whole service/provider’ approach. In doing so they work closely with providers and commissioners to improve the standards of current services.

This includes setting clear outcome focused care plans, taking an innovative approach regarding effective use of current services, and scrutinising services to ensure commissioned support is required and being used appropriately in keeping with a person-centred and strength-based approach. The complex care and review team, work across several areas of social care including Transitions, Learning Disabilities and Mental Health (Borough wide).
