The Physical Therapist Assistant (PTA) provides high-quality physical therapy services to patients in Skilled Nursing, Long-Term Care, and Home Health settings under the supervision of a licensed Physical Therapist. The PTA plays a key role in helping patients improve mobility, strength, balance, and overall functional independence while delivering compassionate, patient-centered care in accordance with established treatment plans and regulatory standards.
Key Responsibilities- Deliver physical therapy interventions as outlined in the patient’s plan of care and under the supervision of a licensed Physical Therapist
- Provide treatment to patients across SNF, LTC, and home health environments, adapting care to meet individual patient needs
- Assist patients with therapeutic exercises, functional mobility, gait training, balance activities, and use of adaptive equipment
- Observe, monitor, and report patient progress, responses to treatment, and any changes in condition to the Physical Therapist
- Educate patients and caregivers on exercises, safety awareness, and strategies to promote independence and prevent injury
- Complete timely, accurate, and compliant documentation in the electronic medical record (EMR)
- Maintain a safe treatment environment and follow infection control and safety protocols
- Collaborate with an interdisciplinary care team to support coordinated and effective patient care
- Ensure compliance with all federal, state, and facility regulations, including HIPAA and clinical practice standards
- Demonstrate professionalism, empathy, and respect in all patient and family interactions
- Graduate of an accredited Physical Therapy Assistant program
- Current Physical Therapist Assistant license in good standing (state-specific)
- Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply)
- Strong clinical, communication, and time-management skills
- Ability to work independently while contributing to a collaborative team environment
