Case Manager
The Case Manager will be responsible for coordinating and managing patient care. This role involves working closely with patients to support their rehabilitation needs. Candidates must possess a minimum of a 2-year degree and a relevant current license. The position requires strong communication skills and the ability to work in a team-oriented environment. Preferred candidates will have a background in clinical liaison or healthcare sales.
The focus will be on adult and geriatric patient populations in a rehab setting. All applicants must adhere to dress code policies during their shift. The position requires collaboration with a range of healthcare professionals.
