Registered Nurse Hospice

About This Role

As a Registered Nurse Case Manager at Elite Hospice, you will take full nursing responsibility for the delivery of the Plan of Care for each hospice patient. This role is ideal for those who are dedicated to providing compassionate care to patients and their families during challenging times.

• Pay: $71,640 - $107,460 annually
• Location:

• Sign-On Bonus: $2,500

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient's and family's physical, psychosocial, and environmental needs and re‑assess as needed, no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, appropriate goals, interventions, and patient or family response to hospice care.
• Collaborate with the patient, family, attending physician, and other members of the IDG in providing patient and family care daily.
• Instruct and supervise the patient and family in self‑care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient’s condition.

• Education:
  
  Current and unrestricted RN licensure in the state of practice
• Experience:
  
  1+ years of clinical experience

  Current CPR Certification required; current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation
