Details and requirements for this opportunity:

* Schedule:

8a-5p, Monday - Wednesday; can be negotiable if providers have different availability.

* Practice Setting:
Outpatient

* Type of cases and required procedures:
Outpatient Hematology/Oncology infusion center

* Credentialing timeframe: 90 days

* Electronic Medical Record (EMR): EPIC

* Certifications required:
Board certification, Basic Life Support

* Licensure required:
Active Oregon medical license and Drug Enforcement Administration license
