PHYSICAL THERAPIST – Home Health Services – Seaside, Oregon
Full time, Day Shift
We are seeking a qualified Physical Therapist to manage and provide skilled physical therapy to patients/clients in a home setting, following a physician's plan of treatment while addressing individual patient needs and maximizing independence.
Hiring Bonus: $7,500 hiring bonus for eligible external hires that meet required qualifications and conditions for payment.
Pay Range: $46.29 – $71.85 per hour (base pay; additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives or bonus opportunities).
Responsibilities• Assess, plan, and implement physical therapy treatment plans for a diverse patient population with various diagnoses and disabilities.
• Conduct patient/client evaluations and provide professional consultations to maximize functional outcomes.
• Coordinate with physicians, interdisciplinary teams, and family members to align care goals.
• Maintain accurate documentation and progress notes in compliance with legal and professional standards.

- Bachelor’s, Master’s, or DPT degree from an accredited Physical Therapy Program
- National Provider BLS certification from the American Heart Association within 30 days of hire
- Oregon Physical Therapist License upon hire
- 1 year of clinical experience in Home Health or a comparable healthcare setting, or active enrollment in a Providence postgraduate preceptor program
