Speech-Language Pathologist – Home Health
Schedule:
4 x 8-hour days
Assignment Details:
- Productivity expectation: 30 points standard
- Patient ratio: 1:1 (may visit up to 6 patients/day)
Requirements:
- Oregon (OR) license required
- 1 year of skilled home health SLP experience
- BCLS certification required
- Oasis experience required
Roles & Responsibilities:
- Provide specialized speech therapy services to patients in a home health setting.
- Assess patient needs and develop individualized treatment plans to improve communication and swallowing disorders.
- Collaborate with healthcare professionals to coordinate care and achieve patient goals.
- Document patient progress and adjust treatment plans as necessary.
- Educate patients and families on communication and swallowing strategies to enhance daily living.
Compliance / Notes:
- Charting systems: EPIC Remote Client for Home Health (laptop), Rover app (phone), and OASIS
- Attire:
Business casual (no jeans); scrubs allowed but not required
