Speech-Language Pathologist – Home Health

Schedule:

4 x 8-hour days

Assignment Details:

• Productivity expectation: 30 points standard
• Patient ratio: 1:1 (may visit up to 6 patients/day)

Requirements:

• Oregon (OR) license required
• 1 year of skilled home health SLP experience
• BCLS certification required
• Oasis experience required

Roles & Responsibilities:

• Provide specialized speech therapy services to patients in a home health setting.
• Assess patient needs and develop individualized treatment plans to improve communication and swallowing disorders.
• Collaborate with healthcare professionals to coordinate care and achieve patient goals.
• Document patient progress and adjust treatment plans as necessary.
• Educate patients and families on communication and swallowing strategies to enhance daily living.

Compliance / Notes:

• Charting systems: EPIC Remote Client for Home Health (laptop), Rover app (phone), and OASIS
• Attire:
  Business casual (no jeans); scrubs allowed but not required
