Physical Therapist – Home Health
Schedule
8-hour day shifts only; weekends not required; no on-call duties.
Assignment Details
Patient load: may visit up to 6 patients per day; guaranteed hours may vary.
Requirements
- Minimum of 1 year skilled home health experience required.
- Current BCLS certification required.
- OASIS documentation experience required.
- Clinical skills in orthopedic post-op, joint replacements, neuro rehab, patient/family education, and home exercise programs are necessary.
Responsibilities
- Provide home health rehabilitation services.
- Conduct skilled assessments and develop treatment plans.
- Educate patients and families on home exercise programs and body mechanics.
- Document patient progress accurately in the required systems.
Compliance / Notes
Travelers are expected to have their own vehicle for patient visits. Parking details and additional compliance requirements will be provided during the onboarding process.
