Position Description

Aging and Disability Services (ADS) works to ensure that older adults and adults with disabilities can live safely, independently, and with dignity in their communities. Through partnerships across local, state, and federal systems, ADS funds and oversees services including case management, in‑home care, caregiver supports, and community‑based living resources. The Care Coordination Program is central to this mission, ensuring clients receive timely, appropriate, and person‑centered support.

Job Responsibilities
  • Lead the development and implementation of a comprehensive performance and evaluation strategy for the Care Coordination Program, including clear outcome measures, KPIs, and accountability structures.
  • Analyze system‑wide data to identify performance gaps and drive targeted improvements.
  • Own and operationalize continuous quality improvement (CQI) efforts, moving beyond recommendations to implementation and measurable results.
  • Partner with providers and internal teams to redesign workflows across intake, assessment, service authorization, and care planning to improve efficiency.
  • Build and maintain dashboards and performance reporting tools that support real‑time decision‑making.
  • Serve as a thought partner to program leadership on strategy, system redesign, and scaling improvements across the network.
  • Integrate equity‑focused evaluation practices, ensuring disparities are identified, understood, and actively addressed through system changes.
  • Lead cross‑functional initiatives to align data systems, workflows, and service delivery expectations.
Minimum Qualifications
  • Bachelor’s degree in Public Health, Social Work, Public Administration, or related field.
  • Minimum of 5+ years of experience in program evaluation, performance management, or system/process improvement within health or human services.
  • Demonstrated experience using data to drive operational or system‑level change.
  • Strong understanding of Medicaid‑funded long‑term services and supports and community‑based care systems.
  • Strong knowledge of aging, disability, Medicaid‑funded services, and Washington State community‑based care systems.
  • Experience with data tools (e.g., Excel, Tableau, Power BI, R, or similar) and translating data into actionable insights.
Desired Qualifications
  • Experience leading end‑to‑end process improvement initiatives (Lean, Six Sigma, PDSA) with measurable outcomes.
  • Experience working within or alongside Washington State long‑term services programs (COPES, TSOA, MAC, etc.).
  • Experience influencing change across multiple stakeholders without direct authority.
  • Strong project leadership skills with the ability to manage multiple complex initiatives simultaneously.
  • Experience integrating equity frameworks into evaluation and operational redesign.
  • Comfort operating in ambiguity and building structure where it does not yet exist.
Additional Information

This temporary assignment is expected to end late November 2027 with a possibility for extension.

The full salary range for this position is $56.57 - $84.88 an hour.
