Job Description UW Medical Center Montlake - Heart Institute has an outstanding opportunity for a Patient Care Coordinator.

Full-time, 100% FTE (40 hours per week) Day Shift Mondays
- Fridays No Telework/On-Site

The Heart Institute at UW Medical Center – Montlake provides a full range of heart healthcare and prevention services. Our services include general cardiology, cardiac diagnostic imaging tests and procedures, cardiac surgery, arrhythmia management and electrophysiology, interventional cardiology, structural heart services, advanced heart failure management, heart transplant and ventricular assist devices (VADs), adult congenital heart disease management, cardiac psychiatry, cardiovascular genetics and other advanced services.

• Triage medical information in a specific medical or surgical specialty
• Schedule post-surgery diagnostic procedures and follow-up patient appointments
• Act as a liaison between patients, consulting physician, referring physician, or outside agency to coordinate patient appointments, admission, or surgery.
• Will coordinate the exchange of information either written or verbally before and after patients are seen
• Coordinate anesthesia
• Obtain all necessary medical records, diagnostic studies, pathology items, etc., that are needed for patient care
• Assist in scheduling new patient appointments by triaging to appropriate department and physician
• Enter appointment information in online appointment system
• Coordinate pre-admission tests and procedures
• Schedule and ensure pre-anesthesia works-ups are completed
• Coordinate the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff, and Admitting
• Other assigned duties

High school graduation or equivalent AND two years of experience in the program specialty OR equivalent education/experience Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration

Pay Range Minimum: $25.58 hourly

Pay Range Maximum: $36.62 hourly
