Social Services Assistant PT Only
Job Summary:
Washington Care Center in Seattle WA is seeking a Social Services Assistant to help the Social Services Director in identifying and providing for each resident’s social, emotional, and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for the resident’s discharge.
Essential Functions:- As part of the admission process, meet with the resident and family to review policies on advance directives, resident rights and non-discrimination.
- Conduct and document a social services evaluation, including identification of resident problems/needs.
- Provide/arrange for social work services as indicated by resident/family needs.
- Assist resident, as needed, with financial planning.
- Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment.
- Meet with the appropriate facility staff concerning resident issues.
- Act in compliance with all corporate, state, federal, and other regulatory standards.
- Access continuing education opportunities appropriate to discipline and responsibilities.
- Assist with discharge planning.
- Works under the direction of the Social Services Director.
- Maintain the confidentiality of all resident care information including protected health information.
- Report known or suspected incidents of unauthorized disclosure of such information.
- Review complaints and grievances made by the resident and make a written/oral report to the Administrator indicating what action(s) were taken to resolve the complaint or grievance. Follow facility’s established procedures.
- Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.
- Participate in resident/group council meetings as requested and provide support services to such council.
- Must have a minimum of an Associate’s Degree and meets State certification requirements.
- A minimum of 1 to 2 years working in geriatrics.
- Preferred:
Experience in Behavior Add-on Programs.
