The Clinical Care Coordinator MA-C will play an integral role in the functioning of Swedish Medical Group s Population Health team, which delivers an improved care experience to patients while enabling the organization to meet its value-oriented/accountable care commitments. The primary responsibility is to deliver on these priorities through patient care coordination, navigation and support of quality workflows. Swedish Medical Group (SMG) provides an extensive range of ambulatory services to patients throughout the Puget Sound.

- Completion of an accredited Medical Assistant program, or completion of a registered apprenticeship program, or completion of post-secondary education with a clinical externship
- Washington Medical Assistant Certification upon hire
- 2 years healthcare experience
- Active national Medical Assistant Certification upon hire (AAMA, AMT, RMA, CCMA or NCCT)
- 3 years work experience in an outpatient/clinic/ambulatory setting
