The Department of Corrections is seeking a highly motivated and qualified individual for the role of Program Specialist 3 SOR-Reentry Case Manager.

The Substance Abuse Recovery Unit (SARU) is responsible for the implementation of evidence-based practices and the operational support to facilitate criminal justice involved individuals' participation in promising practices/programs designed to decrease recidivism which increase staff, criminal justice involved individuals, and public safety.

Working for the Department of Corrections, in collaboration with its Criminal Justice and community partners, this position will contribute to staff and community safety and hold incarcerated individuals accountable by providing case management services to offenders that have been assessed as having an Opioid Use Disorders and Stimulant Use Disorders.

Under the supervision of the Health Services (HS) Substance Abuse Recovery Unit (SARU) Opioid Grant Manager and under the lead of the Program Specialist 4 (PS4), the Program Specialist 3 (PS3) position serves as a coordinator and primary point of contact to support cross-agency release planning and critical transitional services for program participants across the reentry continuum.

The position is responsible for working with the PS4 in providing brief interventions and warm hand off to substance use disorder treatment and/or medication for opioid use disorder services; identify individualized barriers and needs of the participant to enter treatment services; identify community resources and interventions that meet the individualized needs; and to provide tailored, strength-based case management, to include but not limited to linkage to healthcare insurance, follow up to scheduled intake appointments, and referrals to supported housing/employment services, transportation resources, and basic needs if needed.

Administration of case management may occur in prison/jail, community corrections offices, or in the community via by phone/text/email. The PS3 works with program participants, staff, representatives of outside referral organizations and/or members of the public in planning, defining, and developing specialized program activities; monitors and tracks participant eligibility and participation in program activates. The PS3 will identify and recommend resolution for complex problems and issues related to the program.

All activities of the PS3 will directly support the State Opioid Response (SOR) Grant program by providing all the follow-up activities, submitting supporting documentation to the assigned Office Assistant, and record program data in SharePoint Documents and the Funder’s data entry systems to evidence grant deliverables and support program development. The PS3 leads the provision of coordinated linkages to support reentry needs that may include mental and physical health, substance use disorder treatment, employment and education training, housing, transportation, and other services supporting engagement in substance use disorder treatment and/or medications for opioid use.

The objective of the position, thereby, is to reduce the likelihood of participant recidivism, reduce relapse/overdose risk due to opiate and/or stimulant use, and promote a healthy lifestyle of recovery.
