Job Overview

Clinical Pharmacist – Rehabilitation Medicine Services at UW Medical Center – Montlake Pharmacy Department.

Work Schedule
  • Full‑time, 40 hours per week
  • Rotating/Variable shift
Position Highlights
  • Provide extensive pharmaceutical care to patients recovering from cardiac surgery, bone marrow and solid organ transplant, traumatic brain and spinal cord injuries, and involvement with the Neurology service.
  • Play a vital role in the inpatient multidisciplinary patient care team.
  • Support direct patient care, drug therapy consults, protocol development, education, transitions of care, preceptorship of pharmacy students and residents, and perform duties of a Pharmacist 2.
Primary Responsibilities
  • Ensure appropriate drug therapy and nutrition, including indication, dose, interactions, and allergies.
  • Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
  • Use the medical record and other documentation systems to communicate with healthcare professionals and document recommendations.
  • Provide drug information and education to medical teams, nursing, and dietitian staff.
  • Provide patient and family counseling and education; update medication lists and reconcile orders at each visit.
  • Provide primary preceptorship of pharmacy residents and student pharmacists.
  • Perform duties of a Pharmacist 2.
Required Qualifications

• Two years of experience in pharmacy practice. Months spent completing an American Society of Health System Pharmacists accredited residency program may substitute month‑for‑month for the experience.

Legal Requirement

State of Washington Pharmacist License.

Condition of Employment
