Job Description
U W MEDICAL CENTER - MONTLAKE PHARMACY DEPARTMENT is seeking a highly‑motivated CLINICAL PHARMACIST - REHABILITATION MEDICINE SERVICES
.
- Full‑Time / 40 Hours Per Week
- Rotating/Variable Shift
- Unique opportunity to work with Rehab Medicine caring for patients recovering from cardiac surgery, bone marrow and solid organ transplants, and traumatic brain and spinal cord injuries. Involvement with the Neurology service.
- Clinical pharmacists provide extensive pharmaceutical care and are a vital part of the inpatient multidisciplinary patient care team.
- In addition to direct patient care, responsibilities include drug therapy consults, treatment protocol development, patient/family/staff education, transitions of care, precepting pharmacy students and residents, and performing duties of a Pharmacist 2.
Job Responsibilities
- Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
- Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
- Utilize the medical record and other documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
- Provide drug information and education to medical teams, nursing, and dietitian staff.
- Provide patient/family counseling and education; update medication lists and reconcile orders at each visit.
- Provide primary preceptorship of pharmacy residents and student pharmacists.
- Perform duties of a Pharmacist 2.
- Two years of experience in pharmacy practice.
- Months spent in an American Society of Health System Pharmacists accredited residency program may substitute month‑for‑month for the required experience.
- State of Washington Pharmacist License.
