Inpatient Clinical Pharmacist
- Rehabilitation Medicine Service

The UW Medical Center-Montlake Pharmacy Department is seeking a highly-motivated Inpatient Clinical Pharmacist
- Rehabilitation Medicine Service. This is a unique opportunity for a pharmacist to work with inpatient Rehab Medicine which provides care to varied patient populations including those who are recovering from cardiac surgery, bone marrow and solid organ transplant, or traumatic brain and spinal cord injuries. There is also involvement with our inpatient Neurology service.

Our clinical pharmacists provide extensive pharmaceutical care and are a vital part of the inpatient multidisciplinary patient care team. In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, precept pharmacy students and residents, and perform duties of a Pharmacist 2.

Primary job responsibilities include ensuring appropriate drug therapy and nutrition, evaluating pharmacotherapeutic regimens for effectiveness and adverse drug events, utilizing the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations, providing drug information and education to medical teams, nursing, and dietitian staff, providing patient/family counseling and education, updating patient medication lists and reconciling medication orders at each visit, providing primary preceptorship of pharmacy residents and student pharmacists, and performing duties of a Pharmacist 2.

Required position qualifications include two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience. Legal requirement is a State of Washington Pharmacist License.
