Licensed Clinical Social Worker (LCSW) – PRN (W2) in Rehab Without Walls® Home & Community Program, Seattle, WA.

Rehab Without Walls® is seeking an experienced LCSW to join our Home & Community Program in Seattle.

• Provide counseling and psychosocial support in home and community settings
• Help clients navigate real‑life transitions such as returning to work, driving, and increased community participation
• Support emotional, cognitive, and behavioral adjustment following injury or illness
• Collaborate with an interdisciplinary team including OTs, PTs, and SLPs

• PRN W2 position — flexibility without the instability of 1099 work
• Hourly pay (paid for your time, not per visit)

• Perform comprehensive evaluations, assessing the social, emotional and support needs of patients and families
• Set measurable objectives that are formulated in conjunction with the rehab team
• Assist the physician and other health team members in understanding significant social and emotional factors related to the patient's health problems
• Instruct/counsel patients and families in coping with social and emotional responses connected with illnesses
• Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs
• Identify and assist patients/families to utilize appropriate community resources to achieve measurable objectives

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
• CPR certification required
• Ability to lift 50 pounds
• Ability to move, lift, or transfer patients involving up to 100 pounds following safety procedures
• Requires fine motor skills, visual acuity, and walking/standing for extended periods
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently
