Mount Vernon, WA (also Sedro Woolley, Anacortes, Bow) – Service areas: US-WA-MOUNT VERNON, US-WA-SEDRO WOOLLEY, US-WA-ANACORTES, US-WA-BOW

Rehab Without Walls Neuro Rehabilitation is hiring Physical Therapists for our Home & Community Program in Mount Vernon.

Our care model brings outpatient‑quality therapy directly to clients' homes and communities, supporting meaningful, real‑world recovery.

• Deliver functional PT in natural environments
• Assist clients with community mobility, return to work, and daily independence
• Work collaboratively with OT, SLP, and LCSW clinicians

• Perform evaluations and develop treatment plans
• Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes

• Minimum of a bachelor's degree in Physical Therapy from an accredited program
• Current, unrestricted license as Physical Therapist in the state of practice
• Current Basic Life Support or Cardiopulmonary Resuscitation certification
• One year of work experience as a physical therapist preferred
• Knowledge of rehabilitation techniques related to complex neurological injury
• Effective verbal and written communication skills
• Ability to lift 50 pounds; ability to lift up to 100 pounds when following safety procedures
• Fine motor skills, visual acuity, and ability to walk/stand for extended periods
• Physical demands include pushing/pulling, bending/stooping, reaching, kneeling, positioning frequently
