• Perform clinical assessments and determine patient needs and appropriateness for therapy; develop, implement, and revise a Plan of Care (POC) and related progress notes.
• Document procedures, progress notes, plan of care updates, phone orders, and other clinical records; ensure timely and accurate communication to physicians and caregivers while protecting patient confidentiality.
• Assess and intervene to improve range of motion, strength, coordination, and functional mobility; evaluate need for and use of adaptive equipment; modify environments to support safety and ADL performance; fabricate and maintain adaptive equipment and splints as needed.
• Evaluate patient occupational performance across self‑care, work, leisure, and rest; assess sensory‑motor, psycho‑social, and cognitive‑perceptual components of occupational behavior; consider physical, social, and cultural factors of the patient’s environment.
• Apply infection control practices and universal precautions in all patient care activities.
• Accept clinical assignments within education and competency; collaborate with physicians and other health professionals regarding patient status; contribute to a coordinated care plan.
• Maintain accurate records of procedures, organize data, and communicate relevant information to the care team.
• Support compliance with third‑party payers through patient education and coaching as necessary.
• Troubleshoot basic maintenance of occupational therapy equipment and adapt equipment as required.
• Perform other related duties as assigned.

• Current state license as an Occupational Therapist (Washington) – Required.
• Proof of current CPR certification – Required.
• Current driver’s license – Required.
• Reliable transportation and proof of insurance – Required.
• Minimum of two years’ occupational therapy experience in an acute care or rehabilitation setting – Required.
• Education:
  
  Bachelor of Science degree in Occupational Therapy from an accredited program.
• Ability to work in a travel‑oriented, home care environment; flexible schedule and local travel as needed.
