Position: Behavior Health Specialist 254927

We are seeking a compassionate and experienced Behavioral Health Specialist to provide behavioral health services to older adults within an interdisciplinary care setting. This role is responsible for conducting psychosocial assessments, providing therapeutic interventions, coordinating care plans, and supporting participants through complex medical, social, and behavioral health needs. The Behavioral Health Specialist works closely with the Interdisciplinary Team to promote participant well-being, independence, and quality of life.

This is a Direct Hire opportunity.

Responsibilities
  • Conduct comprehensive psychosocial assessments and required six-month reassessments to evaluate participants' behavioral, emotional, social, and environmental needs.
  • Provide individual behavioral health counseling, therapy, and supportive interventions.
  • Develop, implement, and coordinate individualized care plans in collaboration with the Interdisciplinary Team (IDT).
  • Deliver ongoing case management services, including advocacy, crisis intervention, and linkage to community resources.
  • Coordinate transitions of care, including hospital discharges, placement planning, and continuity of services.
  • Support participants and families with advance care planning, end-of-life discussions, grief counseling, and disenrollment processes.
  • Monitor participant progress and adjust interventions as clinically appropriate.
  • Collaborate with medical providers, caregivers, community partners, and support systems to ensure comprehensive care.
  • Maintain timely, accurate, and compliant clinical documentation in accordance with organizational, state, and regulatory requirements.
  • Participate in interdisciplinary team meetings and contribute behavioral health expertise to participant care planning.
  • Ensure compliance with all applicable policies, procedures, and confidentiality standards.
Qualifications
  • Master's Degree in Social Work (MSW) from an accredited institution.
  • Active Washington State Licensed Independent Clinical Social Worker (LICSW) required for Level II compensation.
  • Minimum of 2 years of behavioral health experience; experience working with geriatric populations preferred.
  • Proficiency in clinical documentation and care planning.
  • Ability to work effectively within an interdisciplinary care team and participant-centered care model.

Schedule/ Shift: Monday–Friday, 8am - 5pm
